Personalised Chronic Care Plans

We now offer the new General Practice Chronic Condition Management Plan (GPCCMP) – a Medicare-supported care plan designed to help you manage long-term health conditions. Our team of GPs, nurses, and allied health professionals work together to create a plan that suits your specific needs.

Your care plan may include:

  • Personalised treatment planning with your GP
  • Referrals to allied health providers (e.g., physiotherapists, dietitians, podiatrists)
  • Ongoing support and regular reviews

Medicare Support and Eligibility

If you have a chronic medical condition, you may be eligible for a GPCCMP, which replaces the previous GPMP and TCA system. This plan may give you access to Medicare-subsidised allied health services—usually up to five visits per year, with a rebate of approximately $60 per visit, or in some cases no out of pocket cost to you.
